Reflective case reviews - opportunity to consider any learnings from start of carer journey through to present date
Rapid review - similar to the process for serious case reviews (new process) either a management oversight or small review board to look at whether a full review is required - this will consider and support a "no delay principle" and is a reflective piece of work
- Has there been enough learning?
- Understanding of the situation and what has happened
- Any actions required or taken place to safeguard and reduce repeat?
- ADM - oversight and sign off or request a full review (not in first year)
Forms used - some use the full review docs, if an early review - others created a post-allegation review set of forms - and refer to this in the full review / or post allegation forms - so links and references
Full reviews should be balanced and not all about this situation - not punitive but not dismissive of risks. Post allegation reviews is demonstrating learning and closure to this event. Chronologies in full reviews will link
Panel role - if going through this process is this for info only - no changes but highlighting how robust the Agency has been, or:
- Recommendations required - no change to carers approval terms
- Recommendation - change to terms
- Recommendation - ongoing suitability
Discussion re unplanned ends and disruptions and the learnings from this and how feeds into the review process
Lado process and s47 - safeguarding - this is a separate process and the agency would still do their own review despite any outcome - to satisfy selves of carer suitability, terms and willingness to learn and develop and how the agency can and will support to achieve this
Patterns and themes - considered and may feed into service development
Carers upset that keep bringing up previous allegations or concerns - discussion re balance but how knowing and considering previous concerns might have relevance in any future concerns (if there are any) - otherwise not noted in the review period
